Important Update About Hoag Orthopedic Institute's Patient Portal

Hoag Orthopedic Institute launched a new patient portal, MyChart, with enhanced features that make managing your health more comprehensive and interactive.

Log into MyChart or sign up.

Once your account is activated with MyChart, you can do the following:

  • View your medical information online for discharged patients for Hoag Orthopedic Institute visits after April 29, 2018.
    • Review your medications, immunizations, allergies and medical history.
    • Receive most tests results for tests performed at Hoag, including lab work and imaging.
    • Review after-visit summaries and care instructions pertaining to your hospital stay at Hoag Orthopedic Institute for visits after April 29, 2018.
  • Communicate with your doctor & health care team related to your hospital visit in a secure environment.
  • Manage your appointments and view appointment details for Hoag Orthopedic Institute and a participating Hoag hospital.
  • Access your family's patient records via proxy.
    • Family members can link accounts for convenient access to appointments, discharge care instructions, and more.
  • Pay your bills online.

Our previous patient portal solution through FollowMyHealth, will still be available to access your medical records for any visits made from January 1, 2016 to April 29, 2018.

Your Health Information and Privacy are Protected

Only you (or an approved proxy), your provider and your health care team are permitted to view the information sent through MyChart. We use the latest security practices and safeguards to protect your health care information. MyChart is password-protected and delivered via an encrypted connection.

Signing Up is Easy

MyChart is offered to patients who:
  • Are 18 years of age or older.
  • Have been a patient at Hoag Orthopedic Institute or at a participating Hoag hospital.
  • Have a social security number on file with their provider.

Other Ways to Request Your Medical Records

To obtain medical records copies, a signed authorization is required from the patient or authorized representative*.

*Authorized representative includes parent of a minor patient (under 18 years of age), legal guardian, or is named in the patient’s Advance Health Care Directive as agent to make health care decisions. Proof of representation will be required before records will be released.

If you are unable to download and print the authorization, a written request is acceptable. There is specific information that is required on all requests.

Turnaround Time for Requests

Due to the large volume of requests we receive, your records request will be processed within five to seven business days. Every effort is made to accommodate walk-in requests during normal business hours.

Records will be mailed to where you specify on your request. If you wish to pick up your records, please note that on your request and provide a phone number where you can be reached. Our Release of Information staff will call you when your records are available for you to pick up.

Picking up Copies of Medical Records

Hoag Hospital Newport Beach pick up times:
Monday – Friday (closed holidays), 8:00 a.m. – 5:00 p.m.

Hoag Hospital Irvine pick up times:
Monday – Friday (closed holidays), 8:00 a.m. – 4:30 p.m.

If you plan to pick up copies of your medical records, you must provide photo identification (state driver’s license, state ID, passport, or military ID’s are forms of valid photo identifications) in order to verify we are releasing the information to the correct individual.

If you are unable to pick up your own records and someone else is picking them up for you, you must provide a note giving Hoag Hospital permission to release your information to that person. The individual must then provide their own photo identification at time of pick-up.
